PRIZE CLAIMS: To verify the number on your Scratch & Win Ticket, visit www.dxsrewards.com and follow the online instructions. Once you enter the number online, you will receive an official online confirmation for your prize at the website, and then call the toll-free number on your Scratch & Win Ticket to register your win with Sponsor.

To claim a prize, mail the original, complete winning Scratch & Win Ticket (no copies or mechanical reproductions accepted), your official website confirmation, and a 3x5 card, with your name, mailing address, email address, daytime and evening phone and date of birth, via registered mail, return receipt requested, to: Procaps Direct, DXS Free Paint for a Year Promotion, 14235 John Marshall Highway, Gainesville, VA 20155. Prize claims must be postmarked by 3/1/08 and received by 3/15/08. You should retain a photocopy of the winning Scratch & Win Ticket for your records. Following verification of the winning Scratch & Win Tickets, winners will be notified by mail. All prize claims are subject to verification by an independent judging organization whose decisions are final. Winners may be required to execute an affidavit of eligibility and a liability/publicity release, to be returned within 14 days of date notice is sent or prize will be forfeited and may be awarded to an alternate winner.
